All classes, practice sessions, promotions, tests, contests, seminars and any other such event will be conducted by trained instructors. While reasonable care will be taken to prevent injuries, and minimize accidents, due to the physical nature of martial arts training, members / participants assume all risks associated with any and all events occurring at Team Randori Martial Arts locations or conducted at any other location under promotion of or led by Team Randori Martial Arts owner, staff, representative or assignee. Disclosure of medical conditions to Team Randori Martial Arts does not avail you (or your child if your the parent) of your / their responsibility to gain approval from a competent medical physician nor does it suggest that Team Randori will act, administer, or assume responsibility of treatment or care of know or unknown medical conditions. Furthermore, you agree to waive any and all claims for physical or mental injury or loss of life while at a Team Randori Martial Arts location or event and you understand that Team Randori Martial Arts will not be liable for any harm to you or your children that you represent as the parent or legal guardian.
You warrant and represent that you or the child (children) that you represent are in appropriate physical condition for the physical activities in which you/they will participate. You understand that such physical activities, and even non-contact activities, carry a risk of serious injuries and/or death. Team Randori Martial Arts, has advised you to consult with my physician prior to engaging in any physical activity, and that you should not participate in any activities unless approved to do so by your physician. You knowingly and voluntarily assume all risk describe above, and waive in advance any claim of liability resulting from those risks. On you own behalf and on behalf of my representatives, my estate and all persons claiming through me directly or derivatively. You hereby agree in advance to hold harmless, release and discharge Team Randori Martial Arts and any other owners, officers, employees, affiliate companies, and all other participants and non-participants associate with those identified entities from any injury that you may receive whether or not from the negligent or intentional acts of those persons released. Furthermore, you agree on you behalf and on behalf of your representatives, estate, successors and assigns to defend Team Randori Martial Arts from and against any and all claims and liability that may arise by reason of activities at the facilities, whether caused by an intentional act, or by negligence, or otherwise without limitation. This agreement to defend includes any claim, action or liability whatsoever, regardless of whether the claim is made, without limitation, by you, by someone on your behalf or someone whose class is derived from my injury or death. You acknowledge and agree (1) that it is your responsibility to cover any and all medical bills and associated fees that you may incur and to cover yourself with any medical insurance your may deem appropriate and (2) that you will not and cannot make any claim for medical coverage resulting from any injury or death incurred by you at the facilities of Team Randori Martial Arts, its staff or representatives.
